About The Position

Provides care and treatment to patients on maintenance dialysis. Dialysis treatment is provided within the scope of nursing practice. Home dialysis nurses provide major portions of self-care and home dialysis training for dialysis patients and/or their caregivers.

Requirements

  • Registered Nurse (RN) - Board of Nursing.
  • An active license in the state of practice.

  • Certified Hemodialysis Nurse (CHN) - Board of Nephrology Examiners Nursing and Technology (BONENT).
  • Certified Peritoneal Dialysis Nurse (CPDN) - Board of Nephrology Examiners Nursing and Technology (BONENT).
  • Certified Dialysis Nurse (CDN) - Nephrology Nursing Certification Commission (NNCC).
  • Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI).
  • Dialysis experience or critical care experience.
  • 1-3 years nursing experience if functioning as a home dialysis nurse and an additional 3 months experience in each of the specific modalities (hemodialysis or peritoneal dialysis) for which the nurse will provide self-care training.

Responsibilities

  • Performs and maintains knowledge and proficiency in technical skills for dialysis treatments.
  • Provides oversight and delegation to department staff.
  • Understands and ensures compliance related to water room equipment operation and function.
  • Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental and infection control standards.
  • Documents in the patient record according to departmental policy/procedure.
  • Promotes and maintains documentation and an environment that is in compliance with state, federal, and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.
  • Coordinates care of patients in collaboration with Medical Director, Nephrologist, patients, families, other nursing staff and all ancillary departments.
  • Acts as a patient/family advocate.
  • Collaborates with the patient, family, and health care providers in providing patient care.
  • Identifies patient/family learning needs.
  • Develops, implements, and evaluates an educational plan for patients.
  • Provides patient education related to the dialysis process and procedures related to that process, including care and management of dialysis accesses.
