As the Registered Nurse Coordinator you will work within our Behavioral Health Home located in Princeton, Mercer County NJ. Our Behavioral Health Home integrates behavioral health and primary care utilizing a holistic approach to wellness. The program coordinates clients medical and mental health needs and provides individualized support to help clients obtain and follow through with all aspects of treatment. BHH promotes healthy lifestyles to maintain or prevent chronic medical illnesses. Schedule: Full-time: Monday-Friday 8:30am-4:00pm; 24/7 on call support to program staff and clients; 30 minute unpaid lunch Responsibilities: Supervision and oversight of certified medical staff as well as care coordinators and peer recovery specialist staff in the Behavioral Health Home (BHH); Program development, management and accountability for integration of behavioral and physical health service provision in community-based case management programs; Ongoing facilitation of multi-disciplinary treatment meetings with external and internal stakeholders and providers; Coordination of services with treatment providers; Maintaining strong community relationships for referrals to the program, as well as seamless transitions in care; Continuous quality improvement and performance management reporting; Collection and collation of data for DMHAS reporting annually; Conducting BHH intakes including Nursing Assessment as well as physical health and wellness screenings; Conducting and monitor regular completion of depression and suicide screenings; Facilitation of Social Determinants of Health (SDOH) assessments and integrating results into a holistic wellness plan; Providing care coordination and transition of care for consumer behavioral and physical health needs; Providing linkage to community services that supports holistic needs including basic needs, housing, compliance with behavioral health treatment with a particular focus on integration of physical health services into wellness plan; Ongoing assessment of vitals; Assisting in the creation of person-centered plans; Providing education to client and family including disease management, health/wellness/nutrition, diagnosis; Administering psychiatric injections; Promoting healthy lifestyle choices; Providing 24/7 telephonic support to BHH client and staff, as needed; Engaging in relevant continuing education regarding care coordination and evidenced-based practices for individuals with comorbid behavioral and physical health needs; Other relevant management duties as assigned.

Requirements

  • Bachelors or Associates degree in Nursing or related health field
  • Active, unrestricted NJ license as Registered Nurse
  • Minimum of 2 years’ relevant experience including experience working with persons Severe and Persistent Mental Illness (SPMI), co-occurring substance use disorders (COD) and/or co-morbid physical health conditions
  • Valid Driver's License in good standing
  • All positions require a valid driver’s license in good standing and pre-employment drug screening
  • All candidates, if hired, will be expected to cooperate with Oaks Integrated Care and The Department of Human Services in all inspections and investigations
