Registered Nurse (RN) – Client Assessments, Care Planning & Wellness (Home Care) A Special Touch In Home Care | South Charleston & Huntington, WV Are you an RN who’s ready to step away from the facility grind and use your clinical skill and your people skills to change lives at home? At A Special Touch In Home Care, we’re looking for a Registered Nurse who can confidently assess needs, build strong care plans, and clearly communicate how our services improve outcomes and quality of life. What You’ll Do Client & Prospect Home Visits Conduct in-home assessments for new prospects and current clients Identify risks, opportunities for improvement, and supportive services that can enhance safety and independence Care Plans + Wellness Integration Develop and update individualized care plans based on assessments, goals, and client preferences Ensure our Wellness Plan is embedded into each care plan (as appropriate) to support better outcomes and quality of life Collaborate with office staff and caregivers to ensure care plans are implemented effectively Medication Support Provide medication set up and ensure medication routines are organized and understood Communicate medication-related observations or concerns appropriately to the care team/family Clinical Communication with a Sales Mindset Confidently explain the “why” behind recommended services in a way families understand Articulate how our home care services can reduce complications, improve daily function, and support long-term stability Support conversion of qualified prospects through education, trust-building, and clear next steps Collaboration & Documentation Coordinate with office team and caregivers to ensure care plans are carried out effectively Document assessments, care plans, and updates thoroughly and on time What We’re Looking For Active RN license Experience in home care, case management, hospice, home health, or geriatrics is a plus Strong clinical assessment skills and ability to create practical, client-centered care plans Strong communicator who can connect with clients, families, referral partners, and caregivers Comfortable with a field-based role involving regular home visits Organized, responsive, and able to manage time and documentation efficiently Sales experience or comfort in a consultative/sales-oriented role is strongly preferred (you know how to explain value and help people make decisions) You’ll Thrive Here If You… Love seeing people in their home environment and making practical plans that work Enjoy educating families and building trust quickly Want to be part of an agency that differentiates itself and does just offer “cookie-cutter” care What We Offer Meaningful, mission-driven work with supportive leadership Autonomy in the field with a team that has your back The opportunity to shape how care planning and wellness are delivered across the agency
