Registered Nurse (RN) Case Manager
About The Position
At St. Croix Hospice, the Registered Nurse Case Manager is a central figure in the care model, offering 1:1 holistic support to patients and their families in their homes. This role involves partnering with an interdisciplinary team to create individualized care plans, facilitating end-of-life discussions, and ensuring patient comfort with professionalism and empathy. The company emphasizes compassionate care, guiding patients and families through the end-of-life journey to ensure the highest quality of life, dignity, comfort, and respect. The position is full-time with 100% travel.
Requirements
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience required
- Must have reliable transportation, valid driver’s license, and proof of auto insurance
Nice To Haves
- Hospice/home health experience preferred
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs
- Develop individualized care plans with the patient and family, updating as needs evolve
- Administer medications and treatments as prescribed
- Educate patients and caregivers on care techniques, medications, diet, and symptom management
- Rotate on-call duties and ensure seamless care transitions
