About The Position

Responsible for organizing and providing care to patients through the process of assessment, planning, intervention, and evaluation. Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. This role is accountable for planning effectively in order to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload. Education Qualifications: This position will support Ambulatory (clinic) Setting and the ED. This position requires the ability to work the following schedule: 32-40 hours per week Day Shift 8-4:30pm Holiday Rotation No Weekends Licensure/Certification Qualifications: BSN or ADN degree from an accredited school or college of nursing Current RN licensure in appropriate state(s) of employment Dual ND/MN license required, compensation for dual RN Licensure. 1 year of Case Management Experience Preferred Essentia Health is an integrated health system serving patients in Minnesota, Wisconsin, and North Dakota.   • BSN or ADN degree from an accredited school or college of nursing
  • Current RN licensure in appropriate state(s) of employment
  • Dual ND/MN license required, compensation for dual RN Licensure.
