About The Position

Superior Home Care and Hospice, a locally owned and operated agency celebrating 27 years, is seeking a qualified Hospice and Home Health RN Case Manager. The agency has an excellent reputation for providing high-quality, patient-focused care and prides itself on low employee turnover. This is a part-time, long-term position, flexible at 20-25 hours per week, paid at a per-visit rate. The ideal candidate will share the company's values and assist in providing excellent care to the community. Requirements

Licensed as a Registered Nurse by the State of Utah
Current Utah driver's license and automobile insurance
CPR certification
A minimum of 1 year nursing experience is required

Nice To Haves

Hospice and home health experience is preferred but not required
Passion for providing high quality patient care
Good documentation skills
Enjoy caring for patients in a home setting

Responsibilities

Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care
Create patient centered goals with patients, family members and providers
Assess, supervise and direct patient care
Develop plans of treatment
Ensure chart compliance with state regulations
Coordinate patient care with physicians and other providers
