Requirements
- Current Registered Nurse (RN) license in the state of Nevada (required).
- Strong communication and interpersonal skills, with a deep sense of empathy and connection to patients and families.
- Proficiency in electronic medical records (EMR) systems and documentation.
- Ability to collaborate effectively within an interdisciplinary team.
- Reliable transportation for patient visits (mileage reimbursement provided).
- Comfortable with travel and a flexible schedule.
- COVID-19 vaccination required.
Nice To Haves
- Hospice or home health experience preferred, but not required—we provide training and mentorship.
Responsibilities
- Conduct comprehensive physical, emotional, and psychosocial assessments , developing individualized care plans in collaboration with our interdisciplinary team.
- Provide expert pain and symptom management while supporting the unique needs and goals of each patient.
- Work closely with physicians, social workers, chaplains, and caregivers to ensure seamless, coordinated care.
- Offer compassionate education and emotional support to patients and families, empowering them to make informed decisions about end-of-life care.
- Maintain timely and accurate documentation in electronic medical records (EMR) in compliance with regulatory requirements.
- Participate in interdisciplinary team meetings , contributing insights that enhance patient-centered care.
- Serve as a resource for patients, families, and healthcare providers on pain management, symptom control, and hospice philosophy.
- Engage in ongoing education and professional development to uphold the highest standards of hospice care.
