Department/Unit Summary: A Registered Nurse (RN) case management coordinates comprehensive care for patients, ensuring efficient and effective treatment throughout their hospital stay and facilitating a smooth transition upon discharge. By collaborating with multidisciplinary teams, they develop individualized care plans that address both medical and psychosocial needs. PRN Position

Requirements

  • Education: Associate Degree Nursing (ADN) Required in lieu of copy of Associate's Degree, current RN licensure will be accepted.; Bachelor’s Degree Nursing (BSN) Preferred
  • Licenses: Current RN to practice/work in the state of Arkansas
  • Certifications: Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS) within 1 year of hire; Pediatric Advanced Life Support (PALS) within 1 year of hire; PALS is not required on RNs assigned to the PAIN Clinic. While not always required, obtaining certification such as the Accredited Case Manager ACM credential can demonstrate specialized knowledge in hospital case management.
  • Clinical Expertise: Strong foundation in nursing practices and patient care.
  • Communication Skills: Effective communication with patients, families, and healthcare professionals.
  • Organizational Skills: Efficient management of multiple patients and coordination of diverse care aspects.

Responsibilities

  • Patient Assessment: Evaluate patients' medical conditions, support systems, and financial resources to determine appropriate care plans
  • Care Coordination: Collaborate with physicians, therapists, and nursing staff to ensure patients receive necessary treatments and interventions.
  • Discharge Planning: Develop and implement discharge plans, arranging for follow-up care, home health services, or rehabilitation as needed.
  • Utilization Review: Monitor the appropriateness of hospital admissions and continued stays, ensuring compliance with healthcare regulations and optimizing resource utilization.
  • Patient Advocacy and Education: Advocate for patients' needs, provide education on treatment options, and support informed decision-making.
