Requirements

• Current RN License in the State of Texas or Multi-State Compact License.
• Graduate of an accredited school of professional nursing is required.
• BSN preferred.
• 3+ years RN experience in an acute care setting.
• 2 years of supervisory experience is preferred.
• Case Manager Certification preferred.
• InterQual experience preferred.

Responsibilities

• Promote patient-centered care by coordinating all aspects of hospital-based case management for assigned area.
• Accountable for the overall day-to-day oversight and management of the case management program.
• Function as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the case management team.
• Supervise and monitor professional and support staff to ensure effective care coordination and case management practices.
• Coordinate activities that promote quality outcomes and patient throughput while supporting optimal care and appropriate resource utilization.
