About The Position

We are seeking an experienced RN Case Manager to support care coordination within the Emergency Department . This role focuses on discharge planning, utilization review, and ensuring patients receive the appropriate level of care while optimizing healthcare resources.

Requirements

• Active Registered Nurse (RN) license – Massachusetts
• Minimum 3 years of acute care hospital experience
• Experience in case management, discharge planning, or utilization review
• Knowledge of InterQual® or similar utilization review systems
• Strong communication, organizational, and multitasking skills

Nice To Haves

• BSN (Bachelor of Science in Nursing)
• Case Management Certification (CCM or equivalent)
• Prior experience in Emergency Department case management

Responsibilities

• Assess patients for discharge planning and ongoing care needs
• Coordinate care with physicians, nurses, and interdisciplinary teams
• Perform utilization review using tools such as InterQual®
• Ensure appropriate level of care (inpatient vs. observation status)
• Facilitate insurance authorization and reimbursement processes
• Coordinate patient discharges, transfers, and follow-up services
• Advocate for patients and communicate effectively with families
• Identify high-risk patients and connect them with appropriate resources
• Participate in care rounds and maintain accurate documentation
