As a Registered Nurse Case Manager, you will be the heart of our care model, providing 1:1 holistic support to patients and families in their homes. You'll partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.

Requirements

• Graduate of an accredited school of nursing.
• One (1) to two (2) years of recent acute care experience in an institutional setting.
• One (1) to two (2) years of recent experience in home care preferred.
• Current licensure in State.
• Excellent observation, verbal and written communication skills.
• Problem solving skills and basic math skills.
• Nursing skills per competency checklist.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Provide 1:1 holistic support to patients and families in their homes.
• Partner with an interdisciplinary team to develop individualized care plans.
• Guide end-of-life conversations.
• Ensure comfort with professionalism and empathy.
• Supervise hospice aides.
