About The Position

Care Transition Manager RN Registered Nurse PRN Bring your passion to Texas Health so we are Better + Together · Work location: Texas Health Azle, 108 Denver Trail, Azle, TX 76020 · Care Transition Management department Work hours: PRN, on an as needed basis – prefer 3-4 days per week, flexible coverage 7 days per week: Between Monday-Friday 7:30AM-4:00PM, on weekends between 8:30AM-1:00PM, and ER coverage from 11:30AM-8:00PM. (Does Require 4-weeks of training Monday to Friday 7:30AM-4:00PM) Will cross-train at Texas Health Alliance at 10864 Texas Health Trail, Ft. Worth, TX 76244 Care Transition Management department highlights: · Supporting the 34-bed hospital that has served Azle and the surrounding community since 1954 · Cover Med/Surg, ICU and ER departments · Care Transition is a rewarding career with family-friendly hours Here’s What You Need • Bachelor’s degree in Nursing (BSN) required. (Individuals hired as a Care Transition Supervisor prior to May 11, 2017 will be grandfathered with an ADN at the entity they were employed at on May 11, 2017) • 3 years staff nurse at an acute care hospital required • 1 year discharge planning/care management highly preferred • RN – Registered Nurse upon hire required • CPR – Cardiopulmonary Resuscitation upon hire required and • ACM – Accredited Case Manager upon hire preferred Or • CCM – Certified Case Manager upon hire preferred Or • ANCC upon hire preferred What You Will Do • Working knowledge of medical necessity criteria preferred • Knowledge of Microsoft Outlook and Office (Word, Excel) • Customer service skills • Ability to engage in complex clinical decision-making and discharge planning • Strong oral and written communication skills • Strong commitment to interdisciplinary collaboration • Critical thinking, analysis and conflict resolution skills • Psychosocial and crisis intervention skills • Ability to prioritize and meet deadlines • Identify community resources and service needs and facilitate appropriate referrals as needed. Requirements
