This is an opportunity to transition from traditional bedside nursing to a pivotal role providing compassionate, high-quality care through Telehealth . We are seeking motivated and compassionate Registered Nurses to join our team as a Part-Time Contract RN Care Manager . You will utilize your clinical expertise, critical thinking, and empathy to provide exceptional virtual care and manage the patient journey, ensuring they receive the right care at the right time . This is a Part-Time Contract (1099) role , requiring approximately up to 24 hours per week , offering a competitive rate of $44 per hour . Note: This position does not include benefits.

Requirements

  • RN License Active, unencumbered RN license in NY or, LA, TX, PA is required to start. Willingness to be licensed in all states we serve is mandatory.
  • Minimum 3 years of Registered Nursing experience (hospital exp within Emergency Department is REQUIRED ).
  • Strong critical thinking, outstanding multitasking, and a passion for Telehealth/Virtual Triage.
  • RN Contractors are required to commit to working approximately 24 hours per week . This includes: Two (2) weekend shifts every two (2) months , and One Major and One Minor holiday shift per calendar year.
  • Enthusiasm and savviness for new technology; comfortable documenting in multiple platforms.
  • Reliable, secure high-speed internet (Minimum 20 downloads / 5 uploads).
  • A secure/private area with a door in your home to work from.
  • The ability to work effectively in a work-at-home setting.

Responsibilities

  • Virtual Clinical Care: Provide exceptional clinical virtual care via live phone interactions, applying emergent clinical triage protocols and sound clinical judgment.
  • Patient Navigation: Build trust and rapport with patients and families to navigate them to appropriate care.
  • Aftercare Services: Coordinate and execute aftercare protocols, including timely follow-up on diagnostic and laboratory results , providing patient education, and handling administrative clinical needs.
  • Escalation & Coordination: Manage escalations to Payer Partners and providers and coordinate care seamlessly with our Care Coordinator team.
  • Quality & Documentation: Ensure accurate and complete medical documentation across multiple platforms, focusing on high patient satisfaction and quality outcomes (KPIs include Triage Protocol Accuracy and Patient/Provider Experience).
