About The Position

The Cardiac Surgery Program Nurse is responsible for the administration of a patient-centered and outcomes-driven coordination of care program for all cardiac surgery patients at St. Mary’s Hospital (SMH). Position responsibilities include leading patients and the patient’s support network through preoperative, surgical and post-operative care. The Program Nurse uses evidence-based practice protocols, and clinical nursing judgement to direct processes resulting in improvement of clinical outcomes and processes of care for all cardiac surgery patients. The Program Nurse collects, tracks, and analyzes data related to cardiac surgery patients and implements changes to improve patient experience, decrease length of stay, and lower readmission rates.

Requirements

  • Valid nursing license from the state of practice (required).
  • Associate of Nursing, nursing (required) OR Diploma in Nursing (required).
  • Knowledge of contemporary nursing practice and future innovations.
  • Nursing judgment and skill based on principles of the biological, physical, behavioral, and social sciences.
  • Application of the nursing process in patient care delivery.
  • Coordination of patient care delivery.
  • Evaluation of professional nursing practice to optimize goals and outcomes.
  • Health teaching and health promotion.
  • Patient advocacy and partnership.
  • Interprofessional collaboration and leadership.
  • Understanding and utilization of office and clinical technologies.
  • Compassionate, relationship-based approach to help find meaning in illness, suffering, pain, and existence.
  • Integration of quality improvement, evidence based practices and research in practice.

Responsibilities

  • Conduct initial and ongoing patient assessment in collaboration with the interdisciplinary care team.
  • Analyze assessment data and create a plan of care.
  • Implement treatment and evaluate treatment effectiveness.
  • Administer medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures.
  • Serve as point of contact for patients' care coordination throughout hospital departments.
  • Act as a patient safety advocate by participating in ongoing quality improvement in the department.
  • Incorporate the professional practice model into care delivery.
