Team 12: Albertson, Carle Place, East Hills, East Meadow, East Williston, Elmont, Floral Park, Flower Hill, Franklin Square, Garden City, Hempstead, Herricks, Lake Success, Malverne, Mineola, New Cassel, New Hyde Park, North Hills, Port Washington, Roosevelt, Roslyn, Sands Point, Searingtown, Stuart Manor, Uniondale, Westbury, Queens, Rosedale, Springfield Gardens, St Albans, Woodhaven, Whitestone, & Woodside. 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload Job Details Our Home Care Behavioral Health RN's: Develop a specific behavior health plan of care based upon comprehensive assessment, patient input and consultation with physician and incorporate patient's language, cultural and psychosocial needs into the care plan Coordinates referrals for appropriate services/disciplines/DME/Supplies based upon skilled nursing assessment and physician's orders Assures timely and appropriate reassessment or discharge as per agency requirements Actively participates in the REACT medication reconciliation program . Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment. Provide care to the most upmost standard
Requirements
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse.
- Basic Life Support Certification
- Valid New York State Driver's License and reliable transportation
Responsibilities
