About The Position

Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join their Assertive Community Treatment (ACT) Team. This role involves providing essential, community-based care to adults living with serious mental health and substance use challenges. It is described as impactful, hands-on behavioral health work delivered in homes, shelters, and community settings, rather than traditional facility-based nursing. As an ACT Team RN, the individual will work within a multidisciplinary team, delivering community-based behavioral health support, including psychotropic medication management, holistic nursing assessments, care coordination, and crisis response. The goal is to help individuals achieve stability, independence, and an improved quality of life. The role involves developing meaningful relationships with clients, supporting them through real-life challenges, and ensuring seamless continuity of care across community environments, collaborating closely with psychiatrists, therapists, co-occurring disorder specialists, and other behavioral health professionals to deliver integrated care. Requirements

  • Active, unrestricted RN license in North Carolina
  • Comfort working in community-based settings
  • Valid driver’s license, reliable transportation, and vehicle insurance
  • Strong communication, documentation, and relationship-building abilities

Nice To Haves

  • Experience in behavioral health or mental health (preferred)

Responsibilities

  • Administer and monitor psychotropic medications as prescribed
  • Conduct nursing assessments and track symptoms, side effects, and medical issues
  • Document all care and observations in the Welligent EMR
  • Partner with the team psychiatrist to update medication protocols and care plans
  • Participate in daily in-person ACT Team meetings to ensure coordinated, person-centered treatment
  • Provide crisis assessment and intervention as part of a rotating on-call schedule
  • Deliver nursing care in clients’ homes and community settings
  • Educate individuals on health management, wellness, and medication adherence
  • Advocate for client choice and connect individuals with needed community resources
