Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join their Assertive Community Treatment (ACT) Team. This role involves providing essential, community-based care to adults living with serious mental health and substance use challenges. It is described as impactful, hands-on behavioral health work delivered in homes, shelters, and community settings, rather than traditional facility-based nursing. As an ACT Team RN, the individual will work within a multidisciplinary team, delivering community-based behavioral health support, including psychotropic medication management, holistic nursing assessments, care coordination, and crisis response. The goal is to help individuals achieve stability, independence, and an improved quality of life. The role involves developing meaningful relationships with clients, supporting them through real-life challenges, and ensuring seamless continuity of care across community environments, collaborating closely with psychiatrists, therapists, co-occurring disorder specialists, and other behavioral health professionals to deliver integrated care.
