About The Position

Join Essentia's accomplished team as a Registered Nurse on our Anticoagulation Management Team. We are looking for candidates in Brainerd, MN; Detroit Lakes, MN; Fargo, ND; Duluth MN.

Requirements

• BSN or ADN degree from an accredited school or college of nursing.
• Current nursing licensure in state(s) of employment.
• Basic Cardiac Life Support (BCLS) certification within one month.

Responsibilities

• Provide nursing services in primary and specialty outpatient facilities, non-acute community settings, and through telehealth encounters, ensuring accessible care for all patients.
• Take responsibility for your nursing practice by making informed decisions and promoting health, recognizing that quality care is a universal right.
• Work closely with patients and a multidisciplinary team to assess concerns, identify issues, and integrate both subjective and objective data.
• Monitor and manage anticoagulation therapy, including dosing adjustments based on lab results such as INR, to ensure patient safety and therapeutic effectiveness.
• Educate patients and caregivers on anticoagulation therapy, potential side effects, dietary considerations, and adherence to medication regimens.
• Perform patient risk assessments to identify bleeding or thrombotic risks and adjust care plans accordingly.
• Coordinate with physicians and pharmacists to optimize anticoagulation management and troubleshoot complications.
• Maintain accurate documentation of patient assessments, therapy changes, and communication to support continuity of care.
