Requirements

  • Licensure: Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in the state of Virginia.
  • Experience: Minimum of 2 years of experience in a hospital, long-term care facility, or other related healthcare setting.
  • Home Health Experience: Minimum of 1 year of home health experience required.
  • Communication: Must possess strong written and oral communication skills and the ability to maintain confidential information.
  • Licenses & Certifications: Must have and maintain a valid driver’s license, current CPR certification, and proficiency in Microsoft Office.
  • Skills & Attributes: Compassionate, reliable, organized, and capable of working independently.
  • Strong communication and documentation skills.
  • Physical Requirements: Capable of assisting clients with mobility, performing routine home visits, and lifting up to 25 pounds as needed.

Nice To Haves

  • Previous home care, senior care, or clinical experience preferred.

Responsibilities

  • Conduct comprehensive nursing assessments, develop care plans, and deliver skilled nursing services in accordance with state regulations and company policies.
  • Administer medications, treatments, and injections, monitoring for effectiveness and side effects.
  • Collaborate with interdisciplinary teams—including therapists, aides, and physicians—to ensure seamless care coordination.
  • Educate clients and families about disease processes, medications, and self-care strategies that promote independence.
  • Monitor and document changes in clients’ conditions, promptly communicating concerns to supervisors and other healthcare professionals.
  • Uphold infection control standards, safety protocols, and confidentiality guidelines at all times.
  • Participate in case conferences, staff meetings, and ongoing quality improvement initiatives.
