The Admission Case Manager is an active member of the Patient Care Management and ED team. The role combines every function of case management, from Access through Discharge planning, and post-discharge from the ED. The case manager serves, manages, and supports internal and external customers, communicates appropriately using good interpersonal skills, participates in performance improvement initiatives and programs, participates as a team member, and is accountable for own work responsibilities. Three core responsibilities of the Admission Case Manager are to provide consultation and direction for physicians about the appropriate determination of the level of care, specifically Observation vs. Inpatient status; discharge planning directly from the ED to levels such as Acute rehab, SNF, and homecare services; and development and implementation of care plans for those patients who visit the ED frequently.
