This RN Administrative Coordinator is primarily responsible for reviewing all incoming patient referrals for home health, nursing and therapy services, daily scheduling of home health float staff, and patient care coordination during while a patient is pending their home health start of care or resumption of care. RN Administrative Coordinators are responsible for the programmatic management of Continuum Home Health and Home Infusion. This includes the implementation of the home health program, quality review, quality improvement, intake and referral review and orientation. Nurse Coordinators work closely together as members of the leadership team, and with an interdisciplinary group of clinicians, including RNs, PTs, OTs, SLPs, MSWs, Home Health Aides, and other departments within the agency and UVA Health. RN Coordinators are a local resource for clinical field staff and patients, answering questions related to clinical practice and patient care. Assess the needs of internal/external customers according to established professional standards and institutional guidelines/policy/procedure. Organizes and prioritizes services and activities considering the needs of the customers. Actively participates in quality improvement program development and professional growth activities for self while assisting in the professional development of others. Evaluates effectiveness of services. Demonstrates leadership in the delivery of services. Provides for a safe environment and safe delivery of services.
