About The Position

The Resident Assessment Coordinator (RAC) nurse is a professional registered nurse responsible for implementing health promotion, prevention, and management of sub-acute, chronic illness, and palliative care for Veterans. This role requires leadership and creative approaches in managing acutely and chronically ill patients, ensuring comprehensive and continuous resident-driven holistic care through collaboration with multidisciplinary teams. The RAC nurse also oversees the Safe Patient Handling Program, ensuring safe practices and compliance with health standards.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Proficiency in basic written and spoken English.

Nice To Haves

  • Experience in long-term care quality measures.
  • Knowledge of long-term stay and short-term stay quality indicators.
  • Experience with CMS 5-star rating requirements.
  • Quality Assurance Performance Improvement knowledge under skilled nursing facility guidance.

Responsibilities

  • Implement health promotion, prevention, and management of sub-acute and chronic illnesses.
  • Demonstrate leadership in managing acutely and chronically ill Veterans.
  • Collaborate with multidisciplinary teams to coordinate patient care plans.
  • Oversee the Safe Patient Handling Program and ensure compliance with safety standards.
  • Monitor and improve processes related to safe patient handling.
