St. Mary Rehabilitation Hospital is a 50-bed inpatient rehabilitation facility, a joint venture between Trinity Health Mid-Atlantic and Lifepoint Rehabilitation. Our hospital specializes in caring for patients recovering from strokes, brain injuries, orthopedic trauma, neurological disorders, and other complex conditions. With a team-based, patient-centered approach, we offer personalized care in a supportive, collaborative environment. The Registered Nurse is responsible for the planning and delivery of both direct and indirect patient care through the nursing process of assessment, planning, intervention, and evaluation. You will develop individualized care plans in collaboration with patients, families, and interdisciplinary teams, while promoting safe and effective care that meets the unique needs of our rehabilitation population.

Requirements

  • Graduate of an accredited nursing program (BSN, ADN, or diploma)
  • Current state licensure as a Registered Nurse (RN) in Pennsylvania
  • BLS certification required (American Heart Association preferred)
  • Excellent oral and written communication skills
  • Strong interpersonal and critical thinking abilities

Nice To Haves

  • CRRN certification preferred
  • Minimum six months of medical/surgical experience in an acute care setting preferred

Responsibilities

  • Deliver high-quality, evidence-based nursing care
  • Develop and update patient-centered care plans
  • Monitor and evaluate patient status and respond to changes in condition
  • Collaborate with physicians, therapists, and case managers on care goals
  • Participate in patient and family education
  • Document assessments, interventions, and outcomes accurately and timely
  • Support discharge planning and post-acute care transitions
  • Contribute to a safe and healing environment
