About The Position

Provide professional nursing care to patients experiencing temporary, progressive, or permanent disabilities or illnesses that require rehabilitation. Provide, direct, and supervise nursing care by all nursing staff to meet the needs of patients according to the patient care plans. Maintain the highest degree of quality care.

Requirements

  • Current New York State Registered Professional Nursing License required
  • Current CPR certification required
  • Good organizational, leadership and communication skills
  • Ability to interact with patients and care for their needs
  • Willingness to embrace and exemplify St. John’s Brand characteristics
  • Ability to work effectively in a fast-paced environment
  • Excellent physical and functional assessment skills
  • Technical expertise in MS Word, AOD and charting
  • Technical competencies as defined by the position Skill Checklist to establish and maintain high standards of care
  • Communication of patient care needs with members of other disciplines, utilizing an in-depth knowledge base to ensure comprehensive follow through on problems
  • Ability to compare and interpret previous and present data when problem solving and reporting and recording pertinent data
  • Identification and appropriate reporting of safety hazards
  • Assignment of suitable components of care to LPN’s and CNA’s, to include supervision, teaching and guidance of others as necessary
  • Knowledge based application of group dynamics when working with patients and participating in staff and interdisciplinary meetings
  • Assume responsibility for own professional growth and development

  • Long term care rehabilitation experience preferred

Responsibilities

  • Serve as a clinical resource for those involved in rehabilitation nursing process
  • Participate in the development of evidence-based policies and procedures
  • Demonstrate ability to assess, plan, implement and evaluate nursing interventions through application of a nursing care plan to help patients meet their recovery and rehabilitation goals
  • Collaborate with nurse educators to develop and provide continuing education
  • Collaborate with interdisciplinary team to provide a safe discharge plan and /or transition to another level of care
  • Deliver compassionate nursing care in an environment that seeks to involve patients in each step of their healing process
  • Support patients throughout the recovery process by educating them and helping to establish goals, self-care and effective coping skills
  • Participate in development of care plan with patient, family and interdisciplinary team members
  • Provide direct appropriate nursing interventions
  • Work collaboratively with interdisciplinary care team
  • Identify changes in patient’s condition and intervene within scope of practice
  • Represent nursing at discharge planning meetings and other conferences
  • Provide direct nursing care as needed
  • Maintain proper documentation of records
  • Safely perform direct services to residents such as administering medications and treatments, providing personal care and assistance with activities of daily living
  • Assist at mealtime to ensure an enhanced meal experience for patients
  • Plan and initiate patient and family health teaching and rehabilitation
  • Attend in-service programs provided to facilitate compliance with government regulations and improve quality of patient care
  • Participate in departmental and facility wide quality assurance programs
  • Utilize pager and respond to call lights in a timely manner
  • Actively participate in our Eden Journey
