How you’ll make an impact in this role Clinical Execution & Physician Partnership: Drive clinical excellence by accurately executing complex medical orders and treatment strategies, acting as a high-level partner to physicians in delivering expert, patient-centered care. Interdisciplinary Coordination: Serve as the central communication hub for the interdisciplinary team, ensuring seamless collaboration and the delivery of holistic, well-integrated care journeys. Patient Advocacy & Health Monitoring: Advocate for optimal outcomes by proactively tracking health trends and documenting progress, ensuring the medical team has the insights needed for timely, life-changing interventions. Discharge & Transition Leadership: Lead comprehensive transition planning to empower patients and their families, providing the education and tools necessary for a confident and successful recovery at home.
