Registered Nurse - PRN - Suffolk County
About The Position
Constellation Home Care is looking for a compassionate and skilled Per Diem Registered Nurse (RN) to join our team! This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities. Average schedule is 1 weekend plus 10 visits per month, with a high need for SOC Constellation Health Services was founded to be an innovative leader in the post-acute continuums by the development of core teams that embody clinical excellence, uniqueness within their communities and act with authentic intention. Requirements
- Registered Nurse Licensed in the state of employment, graduate of an accredited RN program.
- At least 1-2 years of RN experience is a MUST; Home care preferred
- Have reliable transportation and be willing to commute to provide the best in-home patient care.
- Some weekend days are required, and summer and winter holiday commitments as needed.
- Basic knowledge of iOs operating system
Nice To Haves
- Oasis experience is strongly desired
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
Responsibilities
- Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
- Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.
- Develop the Plan of Care in coordination with all health care professionals assigned to the case.
- Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Report to the Clinical Manager on a daily basis.
- Provide documentation using the provided iPad within the required time frame.
