About The Position

Perform competent and compassionate care to patients using nursing skills, assessment and education based on knowledge of the nursing process. Coordinate patient care as a team leader practicing Patient-Centered Medical Home principles

Requirements

• Certification or graduation from a four (4) year college or university
• Current KY RN license

Responsibilities

• Monitor patient vitals; detect changes in symptoms, health, and/or pain. Perform duties in compliance with care management and standing clinic orders.
• Fill out and update patient documents and charts.
• Maintain a safe and clean working environment by complying with procedures, rules, and regulations
