About The Position
Heartland Hospice, a member of the Gentiva family, is seeking a compassionate and dedicated Registered Nurse (RN) for a per diem position. This role is essential in providing hospice care, palliative care, and personal home care to patients and their families during critical times. As a Hospice RN, you will be responsible for delivering direct patient care services, managing a reasonable caseload, and maintaining a trusting and compassionate relationship with patients and their families. You will act as the primary liaison between the physician, patient/family, and the hospice team, ensuring that each patient receives the highest quality of care tailored to their individual needs. In this flexible opportunity, you will support patients primarily during weekends, and we encourage you to share your availability so we can accommodate your schedule. Your responsibilities will include evaluating patient and family needs, assisting in the preparation and execution of care plans, and collaborating with an interdisciplinary team that includes Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff. You will also maintain all assigned patient clinical records and participate in quality assessment and performance improvement programs to enhance the care provided.
