About The Position

Registered Nurse - Primary Care Kankakee, IL Full-Time Community Health Partnership of Illinois is a is a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois that specializes in primary preventative medical, dental, and behavioral health service. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve. We are seeking a skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality medical care to patients in a variety of healthcare settings. Your duties will include assessing patients' health, developing and implementing nursing care plans, administering medications, monitoring vital signs, and collaborating with other healthcare professionals to ensure the best possible patient outcomes. The ideal candidate will have a strong clinical background in primary care, exceptional communication skills, and a passion for helping others. A current RN license is required, along with a Bachelor's degree in nursing or a related field. Previous experience in a clinical setting is preferred.

Requirements

  • Active Registered Nurse (RN) license in Illinois.
  • Minimum of 2 years of experience in primary care or a related field.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Proficiency in electronic health record (EHR) systems (Athena Practice preferred).

Responsibilities

  • Deliver high-quality nursing care to primary care patients, including assessments, treatment, care planning, and education.
  • Collaborate with healthcare professionals to develop and implement personalized patient care plans.
  • Administer medications and treatments as prescribed by physicians and in accordance with nursing standards.
  • Monitor patient progress and make necessary adjustments to care plans based on patient needs and responses.
  • Educate and support patients and their families on health management and disease prevention.
  • Maintain accurate and timely documentation of patient care, treatments, and progress in electronic medical records.
  • Participate in quality improvement initiatives and adhere to regulatory and accreditation standards.
  • Maintain confidentiality and protect sensitive patient information at all times.
