The Long Beach VA Health Care System is seeking a few full time Registered Nurse (RN) positions within the Primary Care Department. The PACT RN has duties that include, but are not limited to, providing direct Veteran care and care coordination as it pertains to Veterans and their care within the Primary Care Department. The role involves managing assigned panel(s), utilizing registries, and ensuring appropriate evaluation and access is provided to patient panels. The RN will also provide health promotion, health education, and coaching on wellness, disease prevention, and chronic care management. Additionally, the RN assists in directing the provision of nursing education, orientation, competencies, and providing quality improvement to enhance outcomes utilization. The position requires integrating outpatient nursing knowledge, skills, abilities, and judgment, and being self-directed in goal setting for managing complex PACT situations. The RN will validate understanding of the rationale for care processes and decisions, question clinical practices for the purpose of providing evidence-based care, and demonstrate the ability to cope with and manage competing priorities. Competence in primary care practice and decision-making, deliberate planning, and critical thinking skills are essential. The RN will administer medications and procedures per established policies and guidelines and influence outcomes by collaborating with members of the interdisciplinary team.

  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States.
  • Possession of a current, full, active, and unrestricted registration for graduates of foreign schools of professional nursing.
  • Ability to meet the physical requirements of the job, including heavy lifting and carrying.
  • Experience in primary care nursing.
  • Leadership experience in nursing.
  • Experience with evidence-based practice.
