About The Position

The Registered Nurse for Primary Care Mental Health Integration Coordinator is responsible for providing and supervising direct patient care, focusing on patient education, self-management, and customer satisfaction. This role involves administering medications, collaborating with interdisciplinary teams, and participating in quality improvement processes to enhance care outcomes for Veterans in need of psychiatric nursing care.

Requirements

  • Valid RN License from any U.S. State or territory.
  • Experience in psychiatric care.
  • Master's degree in nursing or related field preferred.
  • Proficiency in English language.

Nice To Haves

  • 5 years of mental health nursing experience preferred.
  • 2 years of primary care nursing experience preferred.
  • Certification in mental health/psychiatric nursing preferred.
  • Experience in interdisciplinary communication and collaboration preferred.

Responsibilities

  • Monitor, assess, provide interventions, and follow physician orders for Veterans in need of psychiatric nursing care.
  • Complete and document nursing assessments within identified parameters.
  • Develop the initial plan of care and provide updates throughout the Veteran's treatment.
  • Work in partnership with interdisciplinary team members, Veteran, and Veteran's family/support system to coordinate inpatient care and discharge plans.
  • Provide age-specific components when planning and providing Veteran care.
  • Consider all characteristics of the Veteran, including age, state of health, race, culture, values, and previous experiences to align care needs.
  • Promote the least restrictive environment that maximizes the Veteran's functional status.
  • Use sound judgment in problem solving and decision making, contacting Supervisor when further guidance is needed.
  • Follow Medical Center policies and procedures, maintaining safety as a priority.
  • Administer medications, monitor for side effects, and document follow-up as outlined by policies and procedures.
  • Follow established safety and infection control standards, policies, and procedures with minimal assistance.
  • Prioritize care for Veterans with the ability to shift priorities based on patient and/or unit needs.
  • Collaborate closely with members of the interdisciplinary team to positively affect patient outcomes.
  • Participate actively in unit/program level quality improvement processes, initiatives, and customer service programs.
  • Effectively communicate with patients, families, peers, and others, adapting style to meet varied educational levels and diverse ethnic backgrounds.
  • Adaptable and able to work in high-stress situations individually or within a team.
  • Deliver care to Veterans and families with VA values.
  • Recognize signs of intoxication and withdrawal, assess suicidal ideation, provide crisis interventions, and connect individuals with appropriate referrals in collaboration with the treatment team.
  • Be organized and responsive to Veteran needs, prioritizing appropriately.
  • Assist with performance of Charge Nurse duties.
