Registered Nurse – Primary Care Case Manager Job Title: Registered Nurse – Primary Care Case Manager Location - Sitka, AS Pay Range - $87,000 to $100,000 Yearly Key Essential Functions & Accountabilities Case Management & Care Coordination Actively manage assigned SEARHC patient panels Collaborate with medical providers and interdisciplinary teams to establish individualized care goals Coordinate multidisciplinary care conferences for high-risk and complex patients Manage acute, chronic, and preventive care needs through EHR reminders and outreach Participate in case management reviews for high-need patients Advocate for patients and families, supporting independence, self-care, and informed decision-making Utilize problem-solving and anticipatory skills to meet patient and provider needs efficiently Referrals & Transitions of Care Coordinate patient referrals within SEARHC and external facilities Track referral progress and outcomes, providing status updates to providers Serve as primary liaison for case managers and discharge coordinators Ensure referral records are received and available for provider review Identify and resolve delays or barriers to care Improve referral coordination processes proactively Clinical Nursing Responsibilities Perform staff nursing duties in clinic settings, including: Patient assessment Documentation Education Medication and therapy administration Participate in triage and respond to emergency situations Maintain annual competencies, including safety and infection control Promote a clean, safe, and well-supplied clinical environment Report defective equipment and safety hazards Adhere strictly to infection control policies Communication & Teamwork Build collaborative relationships across departments and teams Participate in clinic, department, and quality improvement initiatives Serve as a clinical leader among pod staff, nursing staff, and unit coordinators Support ongoing development of the primary care case management role Complete special projects and other duties as assigned Additional Focus Areas Specialty Clinic Coordination Manage specialty clinic referrals and waitlists Review referrals for completeness and appropriateness Coordinate required labs and diagnostics prior to appointments Schedule patients based on clinical priority Coordinate clinic dates and provider availability with outside facilities Track referral outcomes and provide status reports to providers Serve as primary nurse for specialty clinics, providing: Assessment Documentation Education Medication and therapy administration Prepare clinic spaces and assist with procedures (e.g., colposcopy, cystoscopy) Support specialty clinic educational in-services OB Case Management Serve as primary case manager for OB patients Manage intake, tracking, education, and outreach Coordinate care with primary care providers and case managers Track postpartum care and newborn immunizations Telehealth & Diagnostic Coordination Coordinate Video Teleconferencing (VTC) visits with Alaska Native Medical Center specialists Serve as nursing support for telehealth visits Coordinate Exercise Treadmill Test (ETT) referrals and scheduling with Bartlett and SEARHC providers Education, Certifications & Licensure Graduate of an accredited school of nursing (Bachelor’s degree preferred) Current unrestricted Registered Nurse license (Alaska or other U.S. state) Direct hires must apply for Alaska RN license prior to start Agency staff must hold an active Alaska RN license Basic Life Support (BLS) certification required National Case Management certification preferred High school diploma or equivalent required Clinical Competency required within 3 months of hire and every 3 years Experience Requirements One of the following: 1 year general nursing experience plus 6 months as a clinical nurse at a SEARHC outpatient clinic 2 years general nursing experience with at least 1 year in an ambulatory setting 1 year of case management experience Knowledge, Skills & Abilities Knowledge of SEARHC and Alaska tribal health services Strong understanding of the nursing process Customer service-focused approach Knowledge of safety and infection control principles Strong assessment, data collection, and anticipatory skills Proficiency with computers and medical equipment Effective oral and written communication skills Ability to multitask and work independently in a fast-paced environment Strong problem-solving and conflict-resolution skills Ability to anticipate needs and seek assistance when appropriate Ability to stand, bend, stoop, climb, and lift up to 50 lbs Ability to follow complex directions and manage multiple tasks simultaneously Practice nursing using evidence-based models and shared governance Physical Demands Frequent standing, walking, sitting, and use of hands Occasional climbing, stooping, kneeling, crouching, or
