Summary of Essential Duties: As a Transplant Coordinator, Transplant Services, you will assist the Nurse Practitioner/Transplant Coordinator in the coordination of post-transplant care, retrieval of diagnostic work-up, and data entry into CS Link Serves as a patient advocate throughout the transplant process Maintains competency skills including the ability to deliver care to special patient populations and patients with diverse cultural backgrounds Identify the ability of the adolescent, adult and geriatric patients to follow instructions and give consent Identify safety concerns and institute measures to provide a safe environmental and provide appropriate assessment, intervention and evaluation of patients

Responsibilities
