The Patient Navigator for the Suspicion of Cancer (SoC) clinic will provide exceptional, coordinated and patient-centred care that ensures timely access to specialist appointments and procedures. This Patient Navigator follows evidence-based pathways to ensure that SoC clinic patients have quick access to the required diagnostic procedures. The Patient Navigator will remain closely connected with the patient, and the primary care provider to ensure that both are informed about every step of the care plan. Navigating patients during a health crisis is one of the key roles of this position. Reporting to the Oncology Manager, the Patient Navigator will support the patient’s journey through the diagnostic phase of care and will work collaboratively with the interprofessional team. The Patient Navigator will apply his/her knowledge of the health care system and the cancer care system to successfully advocate for the patient/family. The Patient Navigator will be accountable for facilitating the necessary next steps along the care trajectory. Although this position will primarily practice within the Cancer Care and Hematology Program, the navigator will be part of a nursing team supporting other oncology patient navigators and will be expected to provide support as required.
