About The Position

The position is for a full-time Registered Nurse in the Pediatrics Cardiology department at Advocate Health. The role involves providing language-specific education, psychosocial support, and necessary resources for newly diagnosed patients and their families. The nurse will coordinate referrals for needed services, educate patients about their diagnosis, treatments, and community resources, and serve as a liaison between patients and various departments. Responsibilities also include assessing new patients for emotional and social needs, identifying barriers to care, and ensuring patient satisfaction. The working hours are Monday to Friday from 7:00 AM to 3:00 PM, with variable shifts as needed to adapt to patient needs.

Requirements

  • BSN Degree Required, MSN Preferred.
  • Current licensure to practice as a Registered Nurse in the state required.
  • Certification in the appropriate specialized field within 2 years of being in the position required.
  • Excellent communication skills required.

Responsibilities

  • Provides language specific education and psychosocial support for newly diagnosed patients and their families.
  • Coordinates appropriate referrals for needed services.
  • Educates patients and families regarding diagnosis, treatments, and community resources.
  • Serves as a liaison for patients and families between departments.
  • Ensures and coordinates new patient referrals and scheduling of exams and appointments.
  • Contacts every newly diagnosed patient at the time of diagnosis.
  • Assesses new patients for emotional and social needs and barriers to care.
  • Identifies trend barriers to patient expedient care and conducts service recovery as needed.
  • Responsible for patient satisfaction.
