Requirements

  • Associate degree in Nursing (ASN).
  • 1 year of RN experience.
  • Basic Life Support certification.
  • Florida State RN License.

Nice To Haves

  • Care Coordination Experience.
  • Relevant specialty experience.
  • Bachelor's Degree in Nursing (BSN).

Responsibilities

  • Coordinate the care of patients within their specialty.
  • Ensure the full continuum of patient care is being met.
  • Manage new patient consults and patient work up coordination.
  • Schedule continuation of care including autologous donor collection, stem cell lab, One Blood, adult Bone Marrow Transplant team, and others as needed.
  • Coordinate surgeries and act as a liaison with patients, physicians, and other healthcare professionals.
  • Initiate, facilitate, and monitor the development of patient care pathways.
  • Document patient's daily progress in the medical record and communicate unexpected results/symptoms to the physician.
  • Perform nurse visits and wound care to augment care given by the physician.
