About The Position

The Registered Nurse (RN) position at the Lincoln Community Based Outpatient Clinic (CBOC) is part of the Veterans Health Administration's Patient Aligned Care Team (PACT). The RN will engage in a variety of complex nursing activities, collaborating with a multidisciplinary team to deliver comprehensive care to Veterans. This role emphasizes patient-driven health care, focusing on whole person care and the development of personalized health plans.

Requirements

• U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the U.S.
• Graduation from a school of professional nursing approved by the appropriate accrediting agency.
• English Language Proficiency.

Nice To Haves

• 3 years clinical experience with 1 year experience in case management or similar role preferred.

Responsibilities

• Assess, plan, implement, and evaluate care based on age-specific components.
• Administer medications and procedures according to established policies and guidelines.
• Manage assigned patient panels, utilizing registries to ensure appropriate evaluation and access.
• Provide health promotion, education, and coaching on wellness, disease prevention, and chronic care management.
