About The Position

The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing direct care to Veterans within the Primary Care Department. This role involves managing patient panels, promoting health education, and coordinating care to ensure optimal outcomes for Veterans. The RN will utilize nursing assessments to formulate care plans and delegate tasks appropriately, focusing on individual growth and development in outpatient nursing practice.

Requirements

• U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
• Graduation from an accredited school of professional nursing.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state or territory.
• Proficiency in basic written and spoken English.

Responsibilities

• Manage assigned patient panels and ensure appropriate evaluation and access to care.
• Provide gender-specific direct care to Veterans and coordinate their care within the Primary Care Department.
• Deliver health promotion, education, and coaching on wellness, disease prevention, and chronic care management.
• Formulate care plans based on nursing assessments and seek guidance to improve care delivery.
• Delegate elements of care based on assessed needs and patient safety.
