About The Position

The PACT Outpatient Staff RN is responsible for providing optimal nursing care to patients within the Patient Aligned Care Team (PACT) framework. This role involves managing assigned patient panels, ensuring appropriate evaluations and access to care, and delivering health promotion and education. The RN will work collaboratively with the PACT team to implement care plans and support patient needs, while also being accountable for individual nursing practice.

Requirements

• U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
• Full and unrestricted license as a Registered Nurse in any U.S. state or territory.
• Graduation from an accredited school of professional nursing.
• Proficiency in basic written and spoken English.

Nice To Haves

• 1-3 years of outpatient Primary Care, Specialty Care, or Medical Office experience.
• 2-3 years of Triage and/or Critical Care experience.
• Bachelor of Science in Nursing (BSN) preferred.

Responsibilities

• Anticipate and implement preventive patient care measures.
• Manage patient care on the PACT teams and facilitate appropriate care plans.
• Conduct triage, telephone triage, and manage scheduled and unscheduled patients.
• Assess patient status and interpret information to identify patient needs.
• Provide guidance to caregivers and collaborate with the PACT Teamlet regarding care plans.
• Report service needs and contribute to problem resolution.
