About The Position

The Primary Care/Patient Aligned Care Team (PACT) Registered Nurse (RN) Care Manager is a professional nurse, who is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Currently there is one vacancy at VA clinic in Oceanside, California.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • Participation in the COVID-19 vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States.

Nice To Haves

  • Minimum 1-2 year current Registered Nursing experience.
  • 1 year recent clinical experience in caring for patients in Ambulatory Care, Primary Care preferred.
  • Experience with chronic disease management.
  • Ability to work collaboratively with interdisciplinary team.
  • Ability to document using Electronic Medical Records.

Responsibilities

  • Provides direct patient care based on current standards of nursing care, PACT recommendations, patient care needs, and staff competencies.
  • Participates and leads others in PACT performance measure improvement activities such as, but not limited to, 2-day post-discharge contact, chronic disease management, clinical reminder compliance.
  • Participates/proposes/leads others in ongoing PACT process improvement workplace activities as appropriate to the individual's position, education, and practice environment.
  • Plans and leads others in the documentation of patient care and nursing resource utilization, accounting of maximum staff productivity, staff/time efficiency, and positive patient outcomes.
  • Supports more than one PACT as an RN Care Manager when clinically indicated and guides others in the same.
  • Participates in/leads daily and weekly PACT huddles.
  • Performs unit audits as assigned and serves as a Clinic Charge Nurse when applicable.
  • Maintains current knowledge of and adheres to Primary Care standards, recommended practices and guidelines.
  • Completes Care Coordination and Transition Management (CCTM) and Telephone Triage training when available.
  • Completes TEACH and Motivational interviewing training within first year of assignment to PACT.
  • Guides allied health care providers and support personnel with appropriate authority.
  • Performs other duties as assigned.
