Requirements

  • Active Oregon state RN license.
  • Ability to pass a state and national background check.
  • Personal vehicle, valid driver's license, auto insurance, and a clean driving record.
  • 1 year of experience in a home care/home health setting OR at least 1 year in an SNF, ICU, or Med-Surg.

Responsibilities

  • Collaborate with the team to admit clients needing medication administration and nursing services.
  • Perform nursing assessments, monitor client progress, and coordinate care.
  • Delegate nursing tasks to caregivers, ensuring compliance with regulations.
  • Lead the development and updating of individualized care plans.
  • Provide guidance to LPNs and ensure high-quality care delivery.
  • Maintain clear and effective communication with the team and clients.
  • Review client documentation and records, and document nursing services and interactions.
  • Deliver training and education to caregivers to enhance their skills.
  • Participate in company and departmental meetings to stay informed and engaged.
  • Work collaboratively with fellow healthcare professionals to ensure comprehensive care.
  • Ensure compliance with agency policies and regulations.
  • Offer support to other nurses as needed.
