Position Overview Location: Patient visits across San Diego Schedule: Full-Time Compensation: $55–$62/hour (commensurate with experience) This role is community-based, with visits to patients in their homes, assisted living communities, and skilled nursing facilities. As part of our interdisciplinary team, you’ll deliver holistic, patient-centered care that supports both physical comfort and emotional well-being.

Requirements

  • Active, unrestricted RN license in California
  • Minimum of 2 years of nursing experience (palliative/hospice experience preferred)
  • Associate’s or Bachelor’s degree in Nursing
  • Valid driver’s license, reliable transportation, and current auto insurance required

Nice To Haves

  • Community-based or home health experience a plus

Responsibilities

  • Assess patient needs, triage priorities, and deliver skilled nursing care
  • Collaborate with providers, social workers, and spiritual counselors to implement individualized care plans
  • Support emotional well-being and legacy-building activities for patients and families
  • Introduce and coordinate complementary therapies (e.g., massage, music therapy, Reiki, journaling)
  • Facilitate advance care planning conversations and assist with directives
  • Provide education on symptom management and coping strategies to patients, families, and staff
  • Ensure continuity of care across settings and team rotations
  • Document care accurately within RN scope of practice
  • Serve as a resource for clinical teams on palliative care principles
