About The Position

This Registered Nurse position is located in Nursing, Ambulatory Care located at the Edith Nourse Rogers Memorial, Bedford, MA campus. This position is full-time at 40 hours per week. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based care to primary patients. The PACT RN provided direct Veteran care and care coordination as it pertains to Veterans and their care within the Primary Care Department.

Requirements

  • 1 full and unrestricted license from any US State or territory
  • Current and former Federal employees must submit copies of their most recent SF-50 (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held.

Responsibilities

  • Directs the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
  • Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
  • Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others.
  • Participates in precepting new employees (RN, LPN, NAs) and validates unit-specific competency completions
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.
  • Contributes professional outpatient nursing perspectdive in discussions with Interdisciplinary Team.
  • Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
  • Shares educational findings, experiences, and ideas with peers.
