About The Position

The RN Care Manager is an essential component of the PACT Initiative (Patient Aligned Care Teams). The RN Care Manager provides nursing assessment, telephone triage with advice per protocol, patient education and case management responsibilities throughout the health care continuum. Collaboration and networking with multiple disciplines, services and agencies is a necessary component of this role. The RN Care Manager works in collaboration with the Provider and LPN to provide comprehensive care to patients which includes, Telephone Triage, RN Assessment Clinics, Group Clinics, Management of urgent/emergent patient care in the clinic environment, management of patient transitions to the Emergency Department or for inpatient admission, post hospital discharge phone calls to all inpatients discharged from acute care, care coordination, networking with specialty clinics, phone calls to patients to resolve care issues, home management advice, education regarding treatment plan changes, education regarding disease processes, and referrals to community health, HBPC, and CCHT. Individual will be required to provide Clinical Coverage to the other Clinics in time of clinical need. May be assigned to a variety of PACT to include Geri-PACT.

Requirements

  • 1 full and unrestricted license from any US State or territory
