About The Position

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. The nurse performs triage and assessment of patients on the team panel or unit making sure to provide prompt and appropriate treatment for Veterans health concerns to ensure care is timely. The nurse demonstrates high quality customer service skills. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. This includes managing and monitoring high risk patients on the panel for improvement of patient outcomes including prevention of hospitalization and improvement in ambulatory care sensitive conditions. The nurse collaborates and coordinates care with interdisciplinary team members from other services within the department as well as services in the community. The nurse supports virtual care clinic modalities. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care. Shares educational findings, experiences, and ideas with peers. Questions PACT clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture. Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.
