To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum, as needed. The Centers Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes.

  • Align actions with the hospital's mission and vision by providing nursing and case management services to clients and families.
  • Assess patients' clinical and life issues, coordinate care plans, and collaborate with the interdisciplinary healthcare team.
  • Monitor and address symptoms related to oncology and chemotherapy, educate patients and families about their disease and treatment plans.
  • Document assessments, interventions, and patient education in the electronic medical record.
  • Communicate effectively with the healthcare team and patients to ensure coordinated care.
  • Provide patient education and support informed decision-making regarding treatment options.
  • Promote communication and collaboration among healthcare team members, patients, and families.
  • Perform tasks according to facility policies, regulatory requirements, and best nursing practices.
  • Ensure efficient delivery of services by covering for team members and responding to patient calls and drop-ins promptly.
  • Current Georgia RN license from an accredited college.
  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date.
  • BLS (Obtain AHA certification within 30 days of hire).
