About The Position

Colorado PACE has an immediate opening for a Registered Nurse Outpatient Clinic. The PACE Clinic Registered Nurse is responsible for managing the nursing care essential to the assessment, promotion, maintenance and restoration of participant’s health and wellness within their scope of practice. This role focuses on maximizing participant health outcomes and experience by identifying needs, completing pre-visit planning, supporting clinic visits, and providing closure including scheduling follow-up appointments, testing, lab work, and participant instructions. The nurse will also respond to walk-ins and triage calls, and complete nursing visits thoroughly and appropriately. Requirements

  • Graduate of an accredited school of nursing
  • One year of professional experience required
  • Minimum one year experience working with the frail and elderly population, home care or geriatric experience
  • Valid RN License
  • Valid Colorado Driver’s License
  • Proof of Auto Insurance in accordance with state statute

Responsibilities

  • Managing the nursing care essential to the assessment, promotion, maintenance and restoration of participant’s health and wellness within their scope of practice.
  • Maximizing the participant’s health outcomes and participant experience by identifying participant needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits, including scheduling follow up appointments, testing, lab work, and participant instructions.
  • Responding to walk ins and/or triage calls.
  • Thoroughly and appropriately completing clinic visits designated as nursing visits.
  • Demonstrating a working knowledge of best practices in nursing and maintaining professional boundaries.
  • Conferring with team members for effective and timely resolution of participant needs.
  • Maintaining flexibility in schedule and responding to unexpected emergencies and changes in daily assignment to meet organizational needs.
  • Serving in the on-call nurse rotation.
  • Establishing therapeutic relationships with participants and families from a variety of cultural, ethnic, and socioeconomic backgrounds.
  • Demonstrating interpersonal skills with all levels of medical, nursing, administrative, and other interdisciplinary team members to effectively and efficiently provide level of service required by participants.
  • Facilitating communication with community agencies and providers including pharmacies, hospital departments, and extended care facilities to optimize participant transitions and the provision of required care services.
  • Demonstrating exemplary telephone and virtual intrapersonal communication skills to help facilitate the development and nurturing of a caring relationship and experience for a diverse population.
  • Completing nursing responsibilities including but not limited to medication reconciliation, and timely completion of required electronic medical records.
  • Effectively evaluating & communicating care, needs, and providing teaching and instruction to meet participant’s care goals.
  • Providing participant/caregiver education and training specific to identified needs.
  • Responsible for the assessment and treatment of the PACE population.
  • Screening, communicating, and responding to participant needs appropriately.
  • Triaging and responding to participants’ telephone calls using approved protocols.
  • Accountable for direct and indirect quality care for the PACE population.
  • Acting as a content expert and clinic resource by providing staff and participant education.
  • Assisting in the evaluation of medication compliance and the effectiveness of prescribed medications.
  • Assisting in the communication with pharmacy providers.
  • Attending and participating in participant/family meetings, and IDT meetings as requested.
  • Conferring with the Interdisciplinary Team for Plan of Care adjustments.
  • Maintaining confidentiality of all participant events, discussions, and records.
  • Accurately documenting all pertinent information regarding the client in the EMR.
  • Demonstrating compliance with organizational mission, vision and values.
  • Performing clinical duties within the nursing scope of practice adhering to all local, state and federal regulations.
  • Utilizing information management to support decision making to improve participant outcomes, improve health care documentation, assure participant safety and improve performance in participant care, treatment, and services.
  • Documenting provision of care and services in the medical record.
  • Participating in departmental and agency quality improvement activities.
  • Following established procedures, protocols, and standard precautions related to nursing services provided.
  • Properly cleaning durable medical equipment as necessary.
  • Providing for continuity of services in home, community, or clinic through effective information management, both written and verbal communication.
  • Maintaining clinical competency and performing duties within scope of practice.
  • Promoting interdisciplinary collaboration by prioritizing issues and directing communication to the appropriate team members.
  • Ensuring compliance with regulations, professional licensing, and standards of practice.
