Join a highly specialized transplant team where you’ll guide patients through one of the most meaningful journeys of their lives. As the RN Organ Transplant Coordinator, you’ll serve as the key clinical liaison for pre‑transplant patients—coordinating evaluations, educating patients and families, and ensuring seamless communication across the multidisciplinary care team. You will manage the full pre‑transplant process: organizing assessments, reviewing clinical data, maintaining accurate UNOS listings, and supporting patients through education, triage, and personalized care planning. You’ll collaborate closely with physicians, surgeons, dietitians, social workers, and insurance specialists to deliver an exceptional patient experience. This role also plays an important part in quality improvement, process refinement, and maintaining high standards of care across the transplant program.
