About The Position

Helen Porter Rehabilitation & Nursing is a welcoming, secure residence where loved ones are assured of skilled nursing care and assistance with daily living and where privacy is honored, and individuality respected. Nestled in the farmlands of Middlebury, Helen Porter has accommodations for 98 people, is licensed by the state of Vermont and certified by Medicare and Medicaid. Helen Porter is a skilled and intermediate care residence which specializes in providing a home-like environment. We are newly renovated with high ceilings, a beautiful courtyard with a water fountain, flowers, gardening, and pergolas, and circadian lights sparking all throughout the facility. Please note, all hires, including per diem and part time, must commit to a full-time weekday orientation for the first two weeks.

Requirements

• Must be a licensed RN or LPN.
• Commit to a full-time weekday orientation for the first two weeks.
• Ability to work part-time or per diem.

Nice To Haves

• Experience in long-term care or rehabilitation settings.
• Familiarity with dementia care.

Responsibilities

• Provide skilled nursing care and assistance with daily living.
• Ensure privacy and respect for individuality of residents.
• Participate in specialized services for long-term care residents.
• Support individuals in the Memory Care Unit with dignity and purpose.
• Deliver goal-directed care in the Post Acute Unit following surgery or hospitalization.
• Assist with routine therapy services and supervise activities set up by therapy staff.
