About The Position

Boston Mountain Rural Health Center is currently accepting applications for a full time Case Manager at our Huntsville Clinic. Notice: Safety-Sensitive Position This position has been designated as safety-sensitive under the Arkansas Medical Marijuana Amendment. Employees in this role must be able to perform job duties in a manner that ensures the safety of patients, coworkers, and the public. · Employees in safety-sensitive positions are subject to drug and alcohol testing in accordance with company policy and state law. · The use of marijuana (including medical marijuana), illegal substances, or misuse of prescription medications is prohibited for employees in safety-sensitive positions. · A positive drug test result, including marijuana, will result in ineligibility for hire or removal from a safety-sensitive position . The Case Manager will work as part of the clinical team assisting the medical providers in the provision of primary health care. The Case Manager will assist providers by offering health education, chronic disease management education including diabetes education and preventative services to the patient population. The Case Manager will act as a case manager for all chronic diseases. The nurse provides limited direct patient services and performs specifically designated independent procedures as outlined by the nursing scope of practice by the Arkansas State Board of Nursing. Requirements

  • Current Arkansas RN or LPN license
  • Basic Life Support Certification
  • Some Travel Required

Responsibilities

  • Assist medical providers in the provision of primary health care.
  • Offer health education.
  • Offer chronic disease management education including diabetes education.
  • Offer preventative services to the patient population.
  • Act as a case manager for all chronic diseases.
