About The Position

This announcement is seeking a Registered Nurse -RN- Oncology Care Navigator for the VA Pittsburgh Healthcare System located at the University Drive Campus. This RN Navigator will interface with outside referral agencies, and ensure that their business and clinical practices are in compliance with regulatory standards. They will follow patients from earliest suspicion of cancer through the continuum of cancer care. The RN Navigator will participate in the Cancer Care Navigation Team -CCNT Pilot. The pilot will initially focus on conducting a patient needs assessment and developing tools to identify and address potential barriers to cancer care. The pilot will be implemented through a phased approach to facilitate the standardization of patient-driven cancer navigation services for all Veterans. The Cancer Care Navigator is responsible for the development and oversight of daily coordination of care provided to the patient and functions as a liaison between radiologists, referring Physicians, professional health care staff, and other parties involved in the care of Veterans being seen at the Oncology Clinic. The Cancer Care Navigator is also responsible for participating in the Cancer Care Navigation Team pilot.

Requirements

  • Qualified candidate must possess oncology certification or credential from the ONS as per VAPHS policy
  • Licensure: 1 full and unrestricted license from any US State or territory.

  • Preferred Experience: 2 years previous Navigator Experience.

Responsibilities

  • Acts as consultant to inpatient providers ensuring that continuum of care with appropriate patient follow-up
  • Consults home health and hospice care
  • Acts as a liaison to enhance the quality and continuity of care and to expedite appointments for the cancer patient
  • Provides patient/family education
  • Collaborates with multidisciplinary team to discuss/determine the best treatment plan for patient
  • Monitors progress through treatment process, refining and intervening as needed
  • Expedites pathology results to referring physicians
  • Provides materials regarding support groups/centers as appropriate.
  • Maintain own competency and continuing education in cancer screening, prevention and treatment.
  • Assist with clinic walk-ins/phone calls/inquiries including telephone triage to decrease daily interruptions and maintain clinic flow.
  • Post chemotherapy infusion telephone follow-up.
  • Ensure follow-up visit with Oncologist in accordance with individualized treatment protocol requirements
  • Role models effective interpersonal and communication skills.
  • Must be able to independently make decisions in high-stress situations.
